Small Group Proposal Assumptions & Requirements

This proposal is contingent upon the following. Any modifications to these assumptions must be approved by PreferredOne Underwriting.

  • The quoted rates are based on employer contribution level. The employer must contribute a minimum of 50% of the premium for the single rate for all employees electing coverage.
  • Eligible employees must work at least 20 hours per week.
  • Minimum participation requirement is 75% of eligible employees not waiving due to another health benefit plan.
  • Rates are subject to final enrollment including completion of the employer application, member enrollment forms and binder check. All required documents for new sales must be submitted by the 20th of the month prior to the effective date to ensure timely processing and ID card fulfillment. Enrollment cards are issued 15 days after the completed member enrollment forms are received.
  • Rates are based on employee demographic information and employer location submitted to PreferredOne Underwriting. Any changes to the original specifications may result in modifications to our proposal.
  • Groups must maintain at least a minimum of one eligible employee. If enrollment falls below one eligible employee, the group will be terminated.
  • PreferredOne must be the only carrier offered to the group.
  • Proposed rates are guaranteed for 12 months following the inception date noted on the quote.
  • Rates may be subject to additional information.
  • To be eligible for the Connect Network the Employer’s headquarters must be located in the following counties: Anoka, Carver, Chisago, Dakota, Hennepin, Ramsey, Scott, Sherburne and Washington.
  • Any employees residing outside of PreferredOne's network must be approved by the PreferredOne underwriting department and will be subject to the out of network benefits. In network coverage for out of area employees is subject to PreferredOne guidelines.
  • Subsequent premium bills are due on the first of the month.
  • These rates assume all standard benefit plan options.
  • PreferredOne will not be bound by any typographical or clerical errors or omissions contained herein.
