In order to facilitate providing Rx Savings Solutions, PreferredOne will need to use and share certain health information (“HI”) about you and your prescriptions as follows.

• For Payment. We may use or share your HI to determine if prescription drug discounts and payments are available as well as for claims processing. We may also share your HI in order to identify pharmacies with better pricing.
• For Treatment or Managing Care. We may share your HI with your providers to help with your prescriptions and care. We may also share you HI to help you identify generic drugs and medication alternatives.
• For Health Care Operations. We may study data to improve services.
• To Tell You about Health Programs or Products. We may share HI so that Rx Savings Solutions may contact you about their program, other treatments, products, and services. These activities may be limited by law.
