Participating Provider Criteria for PreferredOne Health Delivery Organizations

Health Delivery Organizations include, but are not limited to, Hospitals, Home Health Agencies, Skilled Nursing Facilities, Free-Standing Surgical Centers and Behavioral Health Organizations.

  1. One of the following criteria applicable to each health delivery organizations is required for initial and continuing participation in the PreferredOne network.

Behavioral Health Facilities

  1. Accreditation in Commission on Accreditation of Rehabilitation Facilities (CARF)
  2. Community Health Accreditation Program (CHAPS)
  3. Joint Commission on Accreditation of Health Care Organizations (JCAHO)
  4. Council on Accreditation (COA)

Skilled Nursing Facilities

  1. Continuing Care Accreditation Commission (CCAC)
  2. Community Health Accreditation Program (CHAPS)
  3. Joint Commission on Accreditation of Health Care Organizations (JCAHO)
  4. Accreditation in Commission on Accreditation of Rehabilitation Facilities (CARF)

Ambulatory (free-standing) Surgical Centers

  1. Joint Commission on Accreditation of Health Care Organizations (JCAHO)
  2. American Association of Ambulatory Health Centers (AAAHC)

Hospitals

  1. Joint Commission on Accreditation of Health Care Organizations (JCAHO)

Home Health Care Agencies

  1. Community Health Accreditation Program (CHAPS)
  2. Joint Commission on Accreditation of Health Care Organizations (JCAHO)

Non–Accredited Facilities

Facilities that do not have the required accreditation must provide evidence of:

  1. Centers for Medicaid and Medicare (CMS) approval
  2. Applicable State, Department of Human Services approval
    Pathologists
    Registered Dietictians
    Anesthesiologist
    Neonatologist
