CARRIE'S STORY


Carrie, a new PreferredHealth member, was depressed and struggling to manage her Type 2 diabetes. At the time, she was missing appointments to address her anxiety, neglected taking her medications as prescribed, and wasn't managing her diet. Carrie only sought medical attention when there was a health crisis, sometimes choosing care in an emergency room.

That's when a PreferredHealth Navigator stepped in. Navigators look for patients with chronic conditions and work to eliminate gaps in their care. Working behind the scenes, Navigators reached out to Care Coordinators and Care Teams within the PreferredHealth network clinics on behalf of the patient. Once notified, a Care Coordinator at the clinic contacts the patient, providing information and services to get them on a path to better health. Carrie was identified as needing an extra level of support. The Care Coordinator set-up Carrie with the right network specialists and providers while the Navigator monitored Carrie's compliance to her new treatment plan.

Working with her team, Carrie started to make and keep appointments with the right providers. Her anxiety and depression dramatically improved. An endocrinologist and diabetic educator helped Carrie learn to administer her insulin correctly. Today, Carrie has the knowledge, skills and confidence essential to manage her own health.
