This Customer Notice describes the privacy policies of PreferredOne Community Health Plan, PreferredOne Administrative Services, Inc. and PreferredOne (collectively referred to as “PreferredOne”). We restrict access to nonpublic personal information about you to those employees who need to know that information to provide services to you. We maintain physical, electronic and procedural safeguards to protect your nonpublic personal information.

Customer Only Registration for Personal Claims Data Access
It is our policy to send and receive individualized information through encrypted channels only. If you register to access personal health information, we will send you a Login ID and password via first class mail. Since this Login ID and password provide access to confidential health information, each covered person in a family will be assigned a Login ID and password. If anyone other than you, including a member of your household, uses your Login ID and password to access PreferredOne's secured application site, we are not responsible for such unauthorized access. The first time you use your Login ID and password, you will be required to change your password. Please record this information in a safe place. If you forget your password or misplace it, send us an e-mail requesting a new one.
