INDIVIDUAL PLANS

A Qualifying Life Event allows you to enroll in a PreferredOne individual plan outside the scheduled open enrollment periods, see the chart below.

• Special enrollment qualifying events include loss of Minimum Essential Coverage (MEC)**, marriage, birth, adoption, permanent move into the PreferredOne service area or newly ineligible for premium tax credits and/or cost sharing reductions.
• You must apply within 60 days of the qualifying event.
• Effective dates vary depending on the type of qualifying event.
• Proof of qualifying event must be provided with your application.
• Applications need to be approved by PreferredOne before coverage is effective.
• If you are eligible for financial assistance (tax credits and/or cost sharing) contact MNsure.

How to Apply for Special Enrollment

• If you are not currently a PreferredOne member you must complete an Individual Insurance Enrollment Application form. All Special Enrollment Qualifying Event documentation must be submitted with your enrollment application along with the first month's premium payment. click here to apply.
