Preventive Health Care Services Schedule

The 2016 "PAS Preventive Health Care Services Schedule" and the 2016 "PIC/PCHP Preventive Health Care Services Schedule" (the "2016 Preventive Schedules") are available here.

  • The 2016 PAS Preventive Health Care Services Schedule applies generally to self-insured employer-sponsored group health plans for which PreferredOne Administrative Services, Inc. (PAS) is the third party administrator. However, if you are an enrolled member, please register or login at www.preferredone.com to see the Preventive Health Care Services Schedule that applies specifically to you. Some self-insured employers have customized Preventive Health Care Services Schedules, which apply to their members and are viewable only on their secure member sites. Also, many self-insured employers have fiscal "plan years" (not calendar plan years) that started in 2015 and have not yet ended, and the 2016 Preventive Schedule does not apply to them or to their enrollees.

  • The 2016 PIC/PCHP Preventive Health Care Services Schedule applies to all individual policies that are issued by PIC on or after January 1, 2016.

    The 2016 PIC/PCHP Preventive Health Care Services Schedule also applies generally to group health plans for which PreferredOne Insurance Company (PIC) is the insurer or for which PreferredOne Community Health Plan (PCHP) issues coverage, for their calendar plan years that begin on or after January 1, 2016. However, if you are an enrolled member, please register or login at www.preferredone.com to see the Preventive Health Care Services Schedule that applies specifically to you. Some insured employers have fiscal "plan years" (not calendar plan years) that started in 2015 and have not yet ended, and the 2016 Preventive Schedule does not apply to them or to their enrollees. The Schedule that applies to their members is viewable only on their secure member sites.
