HOSPITAL:
- Leapfrog - Helps consumers make smarter health care choices by showing them what steps hospitals are taking to reduce preventable medical mistakes.
- Joint Commission on Accreditation of Healthcare Organizations (JACHO) - Find a health care organization that meets the Joint Commission's patient safety and quality standards.
- Minnesota Department of Health Adverse Health Events Report - Area hospitals are required to report to the Minnesota Department of Health when any of 27 identified preventable adverse events occur. The first report was released to the public in January 2005.
- Minnesota Hospital Association (MHA) - Includes public reports about medical errors and other adverse events.
- Hospital Compare - A U.S. Department of Health & Human Services tool to provide information on how well hospitals in your area care for adult patients with certain medical conditions. This information compares the quality of care hospitals provide and was created through the efforts of the Centers for Medicare and Medicaid Services (CMS) and organizations that represent hospitals, doctors, employers, accrediting organizations, other Federal agencies and the public.
- Minnesota Hospital Quality Report - This site offers quality information about Minnesota hospitals, and is the product of a joint partnership among the Minnesota Hospital Association, Stratis Health, and more than 100 hospitals from around the state.
HOME HEALTHCARE:
- Home Health Compare - A tool created by Medicare that provides you with information on how well the home health agencies in your area care for their patients.
PROVIDER/PRACTITIONER:
- Minnesota Community Measurement - Collaboration between local health plans and medical groups dedicated to improving patient care in Minnesota.
- NCQA Recognized Physician Directory - This directory identifies physicians who have been "recognized" for performing well in diabetes and heart/stroke programs created by the National Committee for Quality Assurance (NCQA).
- Bridges to Excellence - This organization is a non-profit coalition created to encourage significant leaps in the quality of care by recognizing and rewarding health care providers and practitioners who demonstrate that they deliver safe, effective, efficient and patient-centered care. They provide several consumer tools on their Web site.
