PreferredOne believes that many chronic diseases and medical conditions can be effectively managed
with programs that support and educate patients, their families and medical providers. To meet that need,
PreferredOne offers the following voluntary Disease Management Programs to eligible members. These
programs enable members to make informed decisions about their conditions and help promote healthy
behavior.
- Accordant Care Disease Management for eligible members with chronic, progressive conditions such as Rheumatoid Arthritis, Multiple Sclerosis, Parkinson’s Disease and ALS.
- FREE & CLEAR Tobacco Cessation Program contracted with The Center for Health Care Promotion Inc. uses inbound and outbound telephone support to systematically move eligible members toward successfully quitting tobacco use.
- PreferredOne Healthy Mom & Baby Program self referral maternity management program with the goal of reducing and/or preventing pre-term labor and delivery for eligible members.
- LifeMasters Supported SelfCare Disease Management Program for eligible members diagnosed with certain chronic conditions, such as asthma, heart, lung and artery disease.
A major goal of these programs is to improve the health and satisfaction of eligible members by giving
them a significant role in managing their medical care.
